Registered Nurse

Sun River Health is currently seeking a Registered Nurse to to ensure the overall health and safety of communities through education, advocacy, and delivery of care.
Reports to: Director Public Health Nursing
Required Skills and Qualifications:
- Registered Nurse (RN)
- Associate’s degree in nursing required; Bachelor’s degree preferred
- 1 year of experience in public health
- 1-2 years of experience working with patients and medical providers directly
- Documented proficiency in nursing tasks and achieving positive health outcomes among patients
Job Responsibilities
- Delivery of clinical care on mobile units and in underserved communities
- COVID-19 community initiatives: Vaccination, testing, education
- Public Health Care delivery: Vaccinations, education and screenings, linkage to primary or specialty care
- Performs comprehensive individual and family assessments which include health history, physical assessment, growth monitoring, developmental assessment, psycho-social assessment, assessment of family functioning, assessment for substance abuse or domestic violence issues, and assessment of basic needs including food, housing, income, resources and supports, and access to health care.
- Provides care coordination, referral and follow-up to individuals and families who are members of a vulnerable population and/or high-risk groups.
- Provides education and counseling to individuals, families and community groups that are adapted to their unique needs, lifestyle, cultural and socio-economic situation.
- Collaborates in development and delivery of programs and activities for individuals, families and population groups that promote health and prevent disease, in settings including, but not limited to Health Center sites, mobile units, community organizations and businesses, schools, and the community in general.
- Participates in response teams to prevent, minimize, and contain adverse health events and conditions resulting from communicable diseases; food-, water-, and vector-borne outbreaks; chronic diseases; environmental hazards; injuries; and health disparities.
- Collaborates in the development of and contributes to individual, team, and Departmental quality improvement, clinical competency, performance management and evaluation activities.
- Keeps vehicle equipment operating by following operating instructions; troubleshooting breakdowns; maintaining supplies; performing preventive maintenance; calling for repairs.
- Keeps supplies ready by inventorying stock; placing orders; verifying receipt.
- Documents actions by completing forms, reports, logs, and patient records.
- Complies with organizational policy and procedure, patient confidentiality.
- Updates public health job knowledge by participating in educational opportunities; reading professional publications; maintaining personal networks; participating in professional organizations.
- Follows infection prevention and control procedures. Follows organizational policy for CLIA waived testing
