Case Manager

Sun River Health is a Federally Qualified, Non-Profit Health Center serving communities in Suffolk, Rockland, Orange, Dutchess, Ulster, Sullivan, Columbia and Westchester County. Sun River Health is currently seeking a full-time Genesis Case Manager to join our team at the HUB.
Provides care management services and support to patients with HIV at HRHCare. Links patients to preventive and primary health care, community resources and social services to reduce barriers to care. Provides education to patients to manage their health care. Responsible for proper and timely documentation in the EMR, as well as communication with internal and external referral sources. Provides HIV testing for highrisk individuals and links them to care. Performs other duties as assigned.
Essential Duties and Responsibilities of a Genesis Case Manager
- Conducts comprehensive case management assessments for patients with HIV and other chronic conditions.
- Develops and monitors care plans with patients.
- Refers patient to community services to reduce barriers to care
- Refers those in need of mental health and/or substance use services to the appropriate community resources.
- Works cooperatively and collaboratively with entire HIV Medical and Behavioral Health provider team.
- Facilitates enrollment in HIV Benefit and Entitlement programs.
- Provides appropriate follow up on patient referrals.
- Provides health education, risk reduction and adherence education to patients within the scope of discipline to assist patient in engagement and retention and reduce barriers to care to enhance health outcomes.
- Conducts outreach to community agencies to foster linkage and referral systems for patients.
- Participates in and/or facilitates case conferences, as well as other agency or community based meetings or trainings.
- Maintains case records and documentation as required.
- Participates in ongoing training and regional collaboratives to enhance knowledge and skills.
- Participates in quality improvement trainings and projects.
Minimum Education Requirement: High School Diploma/GED
Minimum Work Related Experience: 2-4 year
